Are you a Carer or do you have a Career – Have you told us

ARE YOU A CARER? – HAVE YOU TOLD US?

DO YOU HAVE A CARER? – HAVE YOU TOLD US?

As a patient of Montgomery-House Surgery knowing that you are a Carer not only allows us to support you and provide you with your annual free Flu vaccination, it also ensures that if you are a Carer to another patient in the Practice we can see that the appropriate consents are obtained to allow you to help them with their medical needs, which could just be helping with their appointments or explaining test results, through to access to their online medical records.

We recognise that there are many Young Carers also that often attend an appointment with the parent they are caring for. Knowing if someone is a Young Carer allows us to support them.

Likewise if you have a Carer, knowing this helps us to support you more.

To let us know that you are a Carer or have a Carer, simply contact the Practice by completing our online form, or give our Patient Services a call.

Our website also has details of organisations that can support you as a Carer. See here. – and select the Carer tab.
