HOW TO REGISTER
AT THE PRACTICE
You can register
at the Practice if you live in our
area. To
register as a new patient, please
complete a form and a health
questionnaire. All of our new
patients are required to attend a
‘new patient’ appointment, so that
we can introduce you to our
practice, establish your current
state of health, check your blood
pressure and offer further services
as appropriate.
